Telephone evaluation and management service by a physician or other qualified health care professional, 5-10 minutes

Alcohol and/or substance abuse structured screening and brief intervention services; greater than 30 minutes

Family Psychotherapy without patient present

Prolonged service(s) in the outpatient setting requiring direct patient contact beyond the time of the usual service; first hour
